RN Case Manager Responsibilities:
- Collaborate with physicians, patients, and families to promote quality outcomes
- Manage patient care across multiple healthcare settings to ensure appropriate levels of care
- Prevent inpatient readmissions by assessing and coordinating resources
- Develop individualized case management plans and track patient progress
- Act as an advocate for patients, ensuring their medical and psychosocial needs are met
- Analyze data to identify patients eligible for the Case Management Program
- Assist with utilization management and ensure compliance with health plan requirements
RN Case Manager Requirements:
- Active RN license in the state of Illinois (in good standing)
- Case Management Certification (CCM) preferred
- Current CPR (BLS) certification required
- Associate’s degree in Nursing required; Bachelor’s degree preferred
- 2+ years of RN experience in a hospital or primary care setting
- Previous case management experience preferred
